This is information gathered before your first appointment so that your RMT can ensure a safe and informed treatment plan. The booking software has an online version of this to complete. If for some reason it is not preferred or not working, please let me know and fill out a printed copy.
Consent Form
This is a form used in person to consent to having work on specific areas. Your RMT will have a printed version of this in the treatment room.
